Instead, 1095-C forms will be <strong>furnished upon request.<\/strong> <em>Please Note that electronic delivery through Self-Service will <strong>still be available<\/strong>.<\/em><br><br><strong>What you need to know:<\/strong><br>&#8211; <strong>What is a 1095-C Form?<\/strong>&nbsp;The 1095-C form provides information about the health insurance coverage offered to you by your employer. It is used to report whether you and your dependents had health coverage during the year.<\/p>\n\n\n\n<p>&#8211;<strong>Why the Change?<\/strong>&nbsp;This change is in line with the Paperwork Burden Reduction Act, which aims to streamline processes, reduce paper waste, and ensure that you receive your forms in a timely manner.<br><br><strong>How to Request Your 1095-C Form<\/strong><br><br>1. <strong>Consent for Electronic Delivery via Self-Service<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Log into <a href=\"https:\/\/ellucianselfservice.wooster.edu\/Student\">Colleague Self-Service<\/a><\/li>\n\n\n\n<li>Navigate to Employee &gt; Tax Information &gt; 1095-C Information<\/li>\n\n\n\n<li>Select &#8220;Receive my 1095-C only in electronic format&#8221;<\/li>\n\n\n\n<li>Save<\/li>\n<\/ul>\n\n\n\n<p>2. <strong>Email Request<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Send an email to benefits@wooster.edu.<\/li>\n\n\n\n<li>An HR representative will respond with a PDF copy of your requested 1095-C.<\/li>\n<\/ul>\n\n\n\n<p>3. <strong>Physical Mail<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Send a written request to the following address:<\/li>\n\n\n\n<li>The College of Wooster<br>Attn: Human Resources<br>1189 Beall Ave<br>Wooster, OH 44691<\/li>\n<\/ul>\n\n\n\n<p>Employees can request a copy of the form, which the employer must provide no later than the later of:<br>&#8211; January 31 of the year following the calendar year for which the return was required to be made (Request for 2025 Form 1095-C can be made up to and on Jan. 31, 2027).<br>&#8211; 30 days after the date of such request.<\/p>\n\n\n\n<p>FAQs regarding the Form 1095-C can be found <a href=\"https:\/\/inside.wooster.edu\/hr\/benefits\/benefits-quick-reference\/\">here<\/a>. An \u201calternate recipient\u201d is any child of a participant (including a child adopted by or placed for adoption with a participant in a group health plan) who is recognized under a medical child support order as having a right to enrollment under a group health plan with respect to such participant is an alternate recipient. Upon receipt, the administrator of a group health plan is required to determine, within a reasonable period of time, whether a medical child support order is qualified, and to administer benefits in accordance with the applicable terms of each order that is qualified. In the event you are served with a notice to provide medical coverage for a dependent child as the result of a legal determination, you may obtain information from your employer; know the rules for seeking to enact such coverage.
