Student Pre-Employment Forms

All employees at the College of Wooster are required to complete new hire paperwork on or before their first day of work.

Name and Social Security Number Verification Form

The Name and Social Security Number Verification Form must be completed using the information provided on your social security card.

Form W-4 , Employee Withholding Allowance Certificate

By completing a W-4 Form your employer can withhold the correct federal income tax from your pay

Form IT-4, Employee’s Withholding Exemption Certificate

An IT-4 Form allows employer’s to determine the number of exemptions, so that the employer can withhold the correct amount of Ohio income tax. Employee’s must also indicate their school district of residence to ensure the employer can withhold the correct school district income tax.

Students with a permanent residence in a reciprocity state (IN, KY, MI, PA and WV):

Form IT-4, Employee’s Statement of Residency in a Reciprocity State

Students with a permanent residence outside Ohio or a reciprocity state:

Form IT-4, Employee’s Withholding Exemption Certificate (Ohio tax form) – omit the fields marked Public School District of Residence and School District No. on the Form IT-4.

Direct Deposit Form

Direct Deposit Verification:

Upon completion for the Direct Deposit Form, the College of Wooster also requires all employees to have their net pay deposited into a checking or savings account. Students must supply the name of the bank, 9-digit routing number, and account number. It is recommended to submit a voided check or a deposit slip with the completed form.

Form I-9, Employment Eligibility Verification

Federal law requires individuals starting employment in the U.S. to complete a Form I-9, Employment Eligibility Verification. Form I-9 will help to verify the employee’s identity and employment authorization.

Students who are U.S. citizens:

Once we have received the documentation from the hiring department for your new job, new hire forms will be emailed to you to complete electronically. When these forms have been completed and submitted electronically, you will then need to schedule a time to come into the Office of Student Employment to show your required identification per page 3 of the I-9 Form.

Students who are non-U.S. citizens:

If you do not currently have a Social Security Number, please contact please contact the International Student Coordinator, so they can assist you with the application process.
Once the Office of Student Employment has received the documentation from the hiring department for your new job, new hire forms will be emailed to you. When these forms have been completed, you will then need to schedule a time to come into the Office of Student Employment to show your required identification per page 3 of the I-9 Form. Also, please bring your passport, I-20 (or other immigration documents), and Social Security Number receipt with you to your appointment. Once you receive your new social security number, you must bring your card into our office as soon as possible.
