Conclusion of Benefits at The College of Wooster

Group Health Plan, Vision Plan, Dental Plan

Coverage terminates the last day of the month of your last day worked. Employees and their dependents enrolled in the health plan, dental plan, and/or vision plan on the day before the date of termination will be offered COBRA continuation coverage. Current COBRA rates are determined by the cost paid by employee and employer plus an additional 2% for administrative cost.

COBRA

Complete instructions on COBRA, as well as election forms, payment information, and other information will be provided by P&A Group, Inc. This information will be mailed directly from P&A Group to your home. For additional COBRA information, please refer to the Group Health Plan Summary Plan Description. Flexible Spending Account Plan – Health Care and/or Dependent Care

If enrolled, the benefit terminates as of your last day actively at work. Employees and their dependents enrolled in the flexible spending account plan on the day before the date of termination will be offered COBRA continuation coverage. Complete instructions on COBRA, as well as election forms, payment, and other information, will be provided by P&A Group, Inc. This information will be mailed directly from P&A Group to your home.

Life Insurance

Insurance coverage will end on the last day worked. You may convert your coverage to an individual life insurance policy. The maximum amount that you can convert is the amount you are insured for under the plan. You may also choose to convert to a lower amount of life insurance. If you are interested in converting your coverage to an individual life insurance policy, please see enclosed packet from Lincoln Financial. Please email Benefits@wooster.edu for the forms to port or convert your policy.

Optional Life Insurance

If enrolled, coverage ceases your last day of employment. You may convert your coverage to an individual life insurance policy without having to provide evidence of good health. Or you may choose a portability option which allows you to continue a certain level of your employee and dependent coverage without having to provide evidence of good health. If you are interested in converting your coverage or choosing the portability option, please email Benefits@wooster.edu for the forms to port or convert your policy.

Long-Term Disability Insurance

If eligible, coverage terminates as of your last day actively at work.

Voluntary Accidental Death & Dismemberment (AD&D) Insurance

If enrolled, coverage ceases the last day of employment. You may convert your coverage to an individual AD&D insurance policy. The maximum amount that you can convert is the amount you are insured for under the plan. You may also choose to convert the current amount of coverage to a lower amount of AD&D insurance. If you are interested in converting your coverage to an individual AD&D insurance policy, please contact Human Resources.

TIAA Retirement Savings

College contributions, if applicable, will cease as of your last day actively at work. For questions regarding your account(s), please contact TIAA’s Participant Information Center directly at  TIAA or 1-800-842-2252.

Unemployment Compensation Notice to Employees

Information you need to file a claim:

  • Your social Security Card
  • Your state issued driver’s license or ID card number.
  • The names and addresses of employers your worked for during the past 18 months along with your quarterly gross earnings.
  • The first/last date of employment with each employer
  • Your most recent employer’s Federal ID number (FEIN) and Employer Account Number (EAN).
  • If you are not a US citizen or national, you will need your Alien Registration card and the expiration date of your work authorization.

Address Updates
Please be sure to keep your address, phone, and email updated with the College. You can make changes by completing the Address Change Form.

Exit Interview
Please complete an Exit Interview to tell us about your time at The College. Please return it to humanresources@wooster.edu.

Keys and Other College Owned Equipment
Please turn keys and College-owned equipment and supplies into your department before departing.

CoW Purchasing Card
College credit cards are canceled on the last day of work. Please contact the Purchasing Office for more information.

Employee Assistance  Program
Access to the Employee Assistance Programs ends on the last date of employment.


Scot Center Access
Scot Center and all other campus building access will terminate on your last date of employment.

Email, Network Access
Your email and network accesses will end on your last date worked.


Parking Pass
Your parking pass is deactivated on your last date of employment.


Identification Card
Your ID card is deactivated on your last date of employment.
